Our client in Brent is seeking an experienced and motivated Social Worker to join their Adult Social Care team.

🔹 Key Responsibilities
- Undertake Care Act assessments, reviews, and support planning for adults and carers
* Deliver strengths-based interventions that maximise independence and reduce reliance on formal services
* Complete Mental Capacity Assessments and Best Interest decisions
* Make referrals and support legal processes, including Court of Protection applications where required
* Participate in safeguarding enquiries, develop safeguarding plans, and apply a Making Safeguarding Personal approach
* Support residents to access community resources, direct payments, assistive technology, and preventative services
* Work collaboratively within multidisciplinary and multi-agency teams
* Screen referrals and participate in duty services, including urgent and emergency assessments
* Act as a Trusted Assessor for equipment and minor adaptations
* Maintain accurate, timely, and high-quality case records and assessments
* Contribute to audits, quality assurance activities, and service development initiatives
