Adult Social Worker - South & West Herts Older People

Hertfordshire are looking for a Social Worker for their Older Peoples - CHC assessments - withing their South & West Herts Older People Services

• Completing Connected Lives assessments for Older People who need access to services,
• Providing information and advice for those over 65 with frailty, physical or mental disability, in line with the Care Act
• Carers assessments for those whose caring role impacts their daily life.
• Supporting with accessing health funding and ensuring that financial assessments are progressed.

You must be a driver with access to own transport.
