Our client in Brighton & Hove is seeking an experienced Social Worker to join their Specialist Community Disability Service (25+).
This is an exciting opportunity to work within a fast-paced Adult Social Care team supporting adults with disabilities through assessments, safeguarding interventions, hospital discharges, and care planning.
Key Responsibilities:
- Undertake Care Act Assessments and person-centred support planning
* Complete reviews of care and support arrangements
* Lead and participate in Adult Safeguarding enquiries and interventions
* Undertake Mental Capacity Assessments (MCA) and Best Interest decision-making
* Complete Domestic Deprivation of Liberty Safeguards (DoLS) work
* Support hospital discharge planning and transitions back into the community
* Respond to placement breakdowns and urgent care needs
* Participate in duty work and respond to referrals in a timely manner
* Work collaboratively with service users, carers, health professionals, and partner agencies
* Maintain accurate and high-quality case records and assessments
