The Complex Care and Safeguarding Team plays a vital role in supporting adults with complex needs, safeguarding concerns, and promoting independence and well-being.
We are currently seeking experienced Senior Practitioners to join this dynamic team and help make a real difference in the lives of Hackney residents.
About the Role:
As a Senior Practitioner, you will:
- Undertake complex assessments and develop outcome-focused care and support plans.
- Lead safeguarding enquiries and contribute to multi-agency risk assessments.
- Work collaboratively with health professionals, housing, and voluntary sector partners.
- Champion strengths-based and trauma-informed practice.
- Support carers and families through advocacy and intervention.
What We're Looking For:
- Qualified Social Workers registered with Social Work England.
- Proven experience in adult social care, particularly in complex care and safeguarding.
- Strong knowledge of the Care Act 2014, Mental Capacity Act, and safeguarding legislation.
- Excellent communication, analytical, and organisational skills.
