About the role:
We are currently seeking a dedicated and experienced Adult's Social Worker to join our Locality Team in Liverpool. As a key member of the team, you will be responsible for providing high-quality social work services to vulnerable adults within the community.
Key responsibilities and duties:
- Conducting assessments and developing care plans for adults in need of support
- Working collaboratively with other professionals and agencies to ensure holistic care for service users
- Providing advocacy and support to empower individuals to make informed decisions about their care
- Participating in case conferences and reviews to monitor progress and adjust care plans as needed
- Ensuring compliance with relevant legislation and best practice guidelines in all aspects of social work
Additional requirements or benefits:
