Children's Senior Practitioner Children Looked After | Shropshire

Our client in Shropshire is looking for a Social Worker or Senior Social Worker to join their Children Looked After Service. This is an excellent opportunity to support children and young people who are looked after, ensuring they receive the right support, at the right time, to achieve positive outcomes.

Key Responsibilities

  • Manage your personal performance and professional development with the support of your Team Manager.
  • Undertake a range of high-quality assessments, informed by direct work and intervention with children and families.
  • Progress children's care plans and pathway plans, ensuring they are accessing the appropriate level of support at the right time.
  • Undertake statutory visits to children placed at home, in foster care and within residential settings in accordance with Children Looked After (CLA) requirements.
  • Manage cases effectively and coordinate a range of meetings, including care planning, risk management, placement stability, Personal Education Plans (PEPs) and review meetings.
  • Provide children and families with appropriate information, advice and support to help resolve difficulties and achieve positive outcomes.
  • Undertake relevant court work, including matters relating to the revocation of care orders, Special Guardianship Orders (SGO) and placement order applications where required.
  • Participate in the team's duty system, responding appropriately to emerging needs and safeguarding concerns.
  • Maintain accurate and up-to-date computerised records and case documentation in accordance with approved policies, procedures and statutory requirements.
  • Work collaboratively with parents, guardians, carers and other professionals, ensuring effective communication and coordinated support.
  • Promote the safety, welfare and stability of children and young people throughout all aspects of case management.
  • Build positive and effective relationships with children and families, ensuring their views and needs are appropriately considered when developing plans and interventions.
  • Work as part of a multi-disciplinary network to ensure children and young people receive the appropriate services and support.
  • Undertake any other duties appropriate to the grading and nature of the role.
