We are seeking an experienced Hospital Social Worker to work at the Royal Free London NHS Foundation Trust (RFL) in the Emergency Department and Medical Assessment and Admission wards for 3 days a week (Friday/Saturday/Sunday).
The purpose of the role is to work as a member of the Discharge Team (medics, nurses and therapists) assessing patients who present at the weekend in the Emergency Department or who have been admitted to the wards. The aim is to prevent admission, re-admission, and reduce the length of stay for patients who are medically stable and can be discharged home with a care package or other support such as PACE (Post Acute Care Enablement). The Social Worker will be working primarily with Camden residents but may be requested to provide information and advice to out-of-borough patients.
