Our client in West Berkshire Council is seeking an experienced Mental Health Social Worker to join its Adult Social Care service. The successful candidate will work with individuals with complex and long-term mental health needs, supporting service users and their carers to achieve positive outcomes and access appropriate care and support.
Key Responsibilities
- Work in accordance with the Health and Care Professions Council (HCPC) Code of Practice.
- Manage a caseload of long-term, complex and challenging cases, including working closely with service users and carers.
- Assess social care needs and eligibility under the Care Act 2014.
- Prepare and submit assessments to relevant health and social care funding panels.
- Support service users and carers to establish appropriate support packages.
- Safeguard vulnerable adults in line with Council and national procedures.
- Apply the Mental Capacity Act appropriately throughout the assessment and support process.
- Undertake safeguarding investigations where required.
- Identify, assess and manage risks using appropriate risk assessment and management processes.
- Maintain accurate, thorough and timely records and reports.
- Promote equality, fairness and dignity in all aspects of practice.
- Participate actively in supervision, appraisal and relevant professional development.
- Comply with West Berkshire Council's health and safety policies, procedures and competency framework.
