Our client, Nottingham City Council, is seeking an experienced Social Worker to join their Disabled Children's Services team, supporting disabled children and their families to access appropriate services and achieve positive outcomes.
This is an excellent opportunity for a Children's Social Worker with experience supporting disabled children and families, strong assessment and report-writing skills, and the ability to work confidently with families, funding panels, and multi-agency professionals. The ideal candidate will be passionate about child-centred practice and ensuring children and families receive the right support to improve their quality of life and achieve positive outcomes.
Key Responsibilities:
- Undertake comprehensive assessments of disabled children and their families to identify needs and determine eligibility for short breaks and Direct Payments.
- Conduct home visits and build effective, trusting relationships with children and families, ensuring their wishes, feelings, and desired outcomes are reflected in plans.
- Review and monitor Direct Payment packages to ensure funding continues to meet assessed needs and deliver positive outcomes.
- Gather and analyse information from families, education, health professionals, and other partner agencies to inform holistic assessments and support planning.
- Produce clear, accurate, and timely reports evidencing the need for short break services and Direct Payments in line with statutory requirements.
- Present assessment and review reports to funding and resource panels, providing clear, evidence-based recommendations and responding to queries.
- Demonstrate a strong understanding of the needs of disabled children, including complex health, developmental, and behavioural needs and their impact on families.
- Work collaboratively with multi-agency professionals to coordinate services, promote inclusion, and improve outcomes for children and families.
- Maintain accurate and up-to-date case records, ensuring assessments, visits, reviews, and reports are completed within required timescales.
- Promote safeguarding and child-centred practice throughout all assessments, planning, and decision-making.
- Travel across and, where required, outside the local authority area to undertake visits and fulfil service requirements.
