We are seeking an experienced adult social worker to join the integrated community service at Trafford Council.
The team works with Adults in a range of situations with varying disabilities, including long-term conditions, adults at risk, drug abuse, Hoarding, dementia and alcohol issues. A significant part of your role will be to provide social work assessment and interventions utilising a strength-based and person-centred approach, and, if necessary, secure appropriate levels of funding where necessary in line with the Reshaping Trafford Policy.
