We are seeking an experienced Social Worker to join the Hospital team at West Sussex, based at St. Richard's Hospital.
The requirement of the hospital Social Work Team is to respond within timescales to assess people both in the hospital and those who have recently been discharged from the hospital. We assist them with arranging any care and support they may need in the longer term to maximise their independence. The work is varied, fast-paced, and there are opportunities to gain experience in working with a variety of professionals, both in the Hospitals and Community Resources including the Voluntary sector.
This role is hybrid with a requirement of 2-3 days present in the office a week, and you will be required to attend the hospital to support with duty and visit the wards to undertake assessments and attend family meetings.
