Social Worker - Hospital Team | Westmorland, Cumbria

We are seeking an experienced social worker to join the Hospital Social Work team at Westmorland and Furness.

This post is for an experienced social worker who will be based across 2 sites which deliver intermediate care on discharge from the hospital.

Intermediate care beds following discharge from the hospital are a short-term, time-limited service designed to support people who are medically fit to leave the hospital but are not yet able to return home safely.
Progress in intermediate care beds following hospital discharge is monitored in a structured, multidisciplinary, and outcome-focused way, with regular reviews against agreed recovery goals.

The role of the SW is to support timely, safe discharge from the IC unit, promote independence, and ensure that long-term needs are appropriately assessed or make a determination for reablement.
Social workers will play a part in overseeing assessment, decision making, risk management, and onward planning and will be part of the Hospital discharge team.
