We are seeking a Social Worker to join the Older People's team at Hertfordshire with knowledge and experience working with older adults and individuals with cognitive impairment
Responsibilities will include:
- Supporting the reduction of the risk of admission to the hospital
- Assessing/reviewing individuals in line with the Care Act
- Arranging appropriate care and support
- Undertaking the Safeguarding Enquiry Officer role
- Completing Mental Capacity and BI decisions
- Working with other professionals
This role is hybrid and core service hours will be Mon-Thurs 9 am-5:30 pm and Fri 9 am-4.30 pm
