We're seeking an experienced Social Worker with a background in supporting adults with physical disabilities and older people to join the Reablement Team at Brent Council.
The Brent Reablement team is an integrated service that delivers fast‑paced responses to hospital discharges, alongside reablement and social care support through a short‑term intervention model. Our team ethos centres on promoting and enabling independence, and we maintain strong working relationships with partners and colleagues across hubs and intermediate care services.
