Social Worker - Young Adults Transitions Team | Bristol

Our client in Bristol City Council is looking for an experienced Adult Social Worker to support adults with care and support needs and their carers.

Key Responsibilities

  • Undertake statutory social work duties in accordance with relevant legislation, policies and professional standards.
  • Assess the needs of adults and their carers, developing, implementing, reviewing and evaluating support plans and outcomes in partnership with individuals.
  • Promote choice, control and independence, supporting people to make informed decisions about how their care and support is organised and how risks are managed.
  • Apply strengths-based practice to help individuals identify and build appropriate support networks to achieve their desired outcomes.
  • Manage an autonomous caseload in line with experience, competence, eligibility criteria and service priorities.
  • Enable self-directed support by undertaking assessments, identifying personal budgets and developing and reviewing support plans in response to changing needs.
  • Advise on and promote the creative use of universal, community and technology-enabled resources, including appropriate equipment and support.
  • Undertake risk assessments and take appropriate action where an individual's safety, wellbeing or liberty may be at risk, in accordance with safeguarding procedures and relevant legislation.
  • Apply the principles of the Care Act 2014, Mental Capacity Act 2005, Mental Health Act 1983 and relevant safeguarding legislation and procedures.
  • Work collaboratively with colleagues across the health and care sector, adopting a multi-agency approach to achieve positive outcomes for individuals.
  • Provide advice and professional support to other adult social care practitioners in relation to the people they are working with.
  • Contribute to the development and supervision of social care workers and undertake student supervision where required.
  • Promote best practice in areas requiring specialist knowledge, including safeguarding adults, mental capacity and mental health legislation.
  • Ensure information is shared appropriately and securely, in accordance with data protection legislation, GDPR and relevant information-sharing protocols.
  • Apply advanced knowledge of relevant legislation, policies and evidence-informed practice to support effective professional decision-making.
  • Maintain professional registration and practice in accordance with Social Work England standards, the Professional Capabilities Framework and the Knowledge and Skills Statement for adult social workers.
  • Undertake an additional specialist role where required, such as Approved Mental Health Professional, Best Interest Assessor, Practice Assessor or Deaf blind Assessor.
  • Participate in relevant rotas, student placements and continuing professional development activities to maintain and develop specialist practice.
